The Primary Health & Wellness Clinics are provided by Nurse Practitioners, Registered Nurses, Social Workers, Registered Dietitians and other health care specialists. A range of services are available such as, Women's Clinics, Diabetic Education Clinics, Foot Care and a variety of Educational Workshops. Fee Schedule Available Upon Request.



Primary Health & Wellness Clinics for Businesses

CPHC Wellness Clinics are available to businesses and other groups in Lanark, Leeds and Grenville and provides health and wellness education programs and service such as annual flu, glucose testing, etc. Fee Schedule Available Upon Request.

CPHC Value For Life

The CPHC Value For Life services are provided across the tri-county of Lanark, Leeds and Grenville. Value For Life is the umbrella name for services required by residents to ensure an environmentally friendly and safe home.

One of the components of the Value For Life Program communicates important medical information to emergency services personnel. The vial, to be kept in the refrigerator, contains health information including demographics, physician and pharmacist contact, allergies and medical information. A Value For Life magnet is placed on the refrigerator door to alert emergency services the client is on the program. There is no fee for this service.

The Optimal Environmental Lifestyle Resource Guide is available upon request.

Lifeline (Serving Lanark, Leeds, Grenville and Kingston)

Life is unpredictable. An unexpected fall or medical emergency can happen at any time leaving a person cut off from the help they need. Lifeline is a personal response service that is available around the clock. A Personal Help Button is worn on wristband or pendant and at the push of a button, one can access prompt assistance from a highly trained Response Centre Associate, even if the individual cannot speak. Lifeline provides peace of mind and safety for thousands of Canadians. There is a fee for this service.

Registered Dietitian Services

Registered Dietitians are considered experts in food and nutrition. We are university educated and registered with the College of Dietitians of Ontario.

We are able to provide you with useful, practical information based on the latest scientific research in the field of nutrition.

We can help you with:

  • Nutrient deficiencies
  • Weight management

We can also help you:

  • Prepare balanced meals
  • Make healthy food choices
  • Read food labels

CPHC Seniors' Exercise Program

CPHC exercise classes are designed to assist older adults and those with physical challenges in maintaining their independence. Classes are geared to personal functional ability. The program complies with the standards and objectives as developed by the Canadian Centre for Activity and Aging (CCAA), a research centre affiliated with the University of Western Ontario, the Lawson Health Research Institution and St. Joseph's Health Care in London, Ontario.

Benefits of participating in the Seniors' Exercise Program include:
- Increased mobility and flexibility;
- Increased sense of control of individual health;
- Decreased risk of falls through improved balance;
- Expansion of social network through interaction with other seniors;
- Social events and friendships arise through participating in this program;
- Estabalishment of links with other services offered by CPHC and other organizations.

- 50 years +;
- Classes geared to individual fitness abilities;
- Classes take place at least once a week at identified sites throughout Lanark, Leeds and Grenville;
- Individuals are encouraged to work out at their own pace, and only do what they feel they are able;
- Participants workout with music for an enjoyable experience;
- All instructors are certified by the Centre for Activity and Aging, University of Western Ontario.

In-Home 10 Step CPHC Seniors' Exercise

Exercise program designed for seniors or those adults physically compromised that cannot get out of their homes. A trained CPHC Volunteer Exercise Instructor is matched with the senior (one on one) to teach them a 10-Step exercise program in their home.  There is a nominal fee for this service.

Diabetes Education

Individuals living with diabetes learn to manage their condition, enabling them to live healthier lives. Participants learn how to test their blood sugars, recognize and manage low and high blood sugars, plan meals, cope with stress, exercise to control blood sugars as well as management and prevention of diabetes related complications. Educational classes are provided during the day and cover a variety of topics essential to diabetes management. Other services available at the Education Centre include counselling, regular follow-ups, insulin starts and follow-ups, Gestational diabetes, pre-diabetes and those at risk. The Diabetes Team accepts self-referrals and referrals from physicians and other health and social service providers.

Private foot care provided in the home by a professional, for individuals who are e unable to leave their home. There is a fee for this service.

Paternity Testing

Paternity/DNA Testing provides confirmation or exclusion of a genetic relationship. Testing is conducted in a confidential environment and is simple, painless and non-invasive. There is a fee for this service.
