Caregiver Support Services (Alzheimer Outreach)

The Alzheimer Outreach Program began in 1989 serving the tri-county of Lanark, Leeds and Grenville. This program assists the client, the caregiver, and their families an offers a variety of services tailored to each individual's particular needs.

Common Symptoms of Alzheimer Disease

  • Gradual memory loss beginning with most recent events
  • Loss of ability to undertake routine activities
  • Difficulty in learning new things
  • Impaired judgement
  • Loss of ability to recognize family and friends
  • Confusion and Disorientation
  • Loss in language skills

Regular contact is made between the caregiver, a CPHC Coordinator and if appropriate, the client to ensure all needs are being met.  CPHC Coordinators and Respite Caregivers are specially trained in handling the needs of individual's with Alzheimer Disease and related dementias, the frail elderly and those with physical disabilities. 

Caregiver Support Services  include:

  • Music and Memories Luncheon
  • Support Groups
  • Caregiver Support, Information, Education, Training and Counselling

 
In-Home Respite Program

Caring for a loved one with Alzheimer Disease or a related dementia is a difficult 24/7 challenge. This results in stressful situations where the caregiver often cannot leave the home for routine activities, medical appointments, shopping, banking, etc.  A respite worker comes into the home to relieve the caregiver of their care giving role and to help relieve stress placed on family relationships.  The respite worker provides stimulation appropriate to client's interests, activation and socialization.  During this interval, the caregiver can freely attend to any activities with peace of mind.

Music and Memories

Music and Memories is a wonderful opportunity to socialize, sing and reminisce. Music has been deeply shown to stimulate the mind and is deeply rooted even as memory loss takes place. Each session is approximately 3 hours in duration and is ideal for those clients with memory impairment that are overwhelmed by a day-long program.
 

 

Caregiver Support & Counseling - Support Groups

The CPHC Alzheimer Outreach Program facilitates support groups jointly with the Alzheimer Society.  Support Groups bring caregivers together to exchange ideas and share experiences. 

 

 

Various events are organized throughout the year for the caregivers such as:

  • Caregiver Getaways
  • Ladies' Picnic
  • Mens' Breakfast
  • workshops

Individual counselling is also available.

      

 
Information, Education and Training

Emotional support is provided to the caregiver and/or family members through home visits and telephone support.  We provide information on the disease process, available resources, assistance in problem solving, coping skills and making plans for the future. CPHC Coordinators are knowledgeable about community services that support caregivers and/or family including information and advocacy.

 

Adult Day Services

This community-based respite service is designed to meet the needs of physically disabled and/or cognitively impaired adults.  Social and recreational needs are met within supervised, safe and secure environments at various locations throughout Leeds & Grenville.
